INSTRUCTIONS

1. Upload all files (abstracts, biographies, cover letters, Copyright Release form*).  The uploaded files will appear in the Submitted Files box once they are uploaded.

2. The Copyright Release form is now a fillable PDF. Download and fill it in using Adobe reader, save and return. No scanning or faxing required. 

3. Complete the Presentation, Contact and Presenter information sections.

4. Verify the checklist.

5. Click on Submit at bottom of the form.
